11/25/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C conducted an individual session reviewing Ashiyah behavioral progress, future goals, and struggles. Upon arrival Ashiyah appeared to be in a calm and in a relax manner evident by her lack of fidgeting and relaxed body language. QMHP-C engaged Ashiyah in a discussion to determine any new behaviors since the last session. She reported there were no new behaviors that she needed to discuss with QMHP-C. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C during the session. Ashiyah struggled 1. Intervention: QMHP-C questioned Ashiyah on her current emotions as well as her trigger for emotions. QMHP-C utilized open discussion to review Ashiyah’s week thus far including her behaviors and interaction with family and community. QMHP-C presented Ashiyah with a behavioral sheet and ISP and reviewed her ISP with as a way to assist her with understanding what her goals are and how she can achieve these goals on a daily and weekly basis. QMHP-C reviewed the purpose and importance of the ISP with Ashiyah. 2. Intervention: QMHP-C utilized open discussion to review Ashiyah’s ISP verbalizing observed progress and limitations with each goal. QMHP-C presented Ashiyah with paper and writing utensils to draft up her current long term and short-term goals as well as what she would like to get from the services and what she has gained since she has been receiving services. QMHP-C encouraged Ashiyah to point out stressors which could deter her from her goals and assist her coming up with plans on how to avoid and deal with those stressors. 1. Response: Ashiyah reported everyday normal emotions of content but had no trigger for her emotions to report. Ashiyah reported appropriate and respectful behaviors but explained that her mother and her got into an argument to she was currently avoid her because of frustration with her. Ashiyah marked appropriate behaviors and no negative behaviors even though she attempts to discord with her mother when they occur. Ashiyah listened without interrupting or questioned QMHP-C on her information. Ashiyah verbalized high progress with all her goals even though she still struggles with a lot of the behaviors still on the ISP. 2. Response: Ashiyah was unable to list out her goals and needed modeling from QMHP-C to come up with her own examples. Ashiyah admit to limitations within herself and stated that she just wanted someone help her change her attitude without change her personality. Ashiyah pointed out stressors of her mother and girls she disliked both at school and in the neighborhood. QMHP-C Interpretation of client’s progress towards today’s objectives: Ashiyah continues to struggle with being able to express her feelings and emotions in an assertive manner. QMHP-C Plan of Action: VACCD staff will continue to meet with Ashiyah to focus on her relationships with her peers and siblings in the home, school and community.
